Blood Cancer Diagnosis 'Spot Leukaemia' Workshop

Continuity of care in General Practice – why is it important and how can we improve it?

The Continuity of Care webinar will showcase the projects that were chosen for the Health Foundation’s Increasing Continuity of Care programme. This past January, five teams began implementing projects which use improvement approaches to support an increase in continuity of care in their GP practices. During the webinar, we will discuss the value of continuity of care in general practice before hearing from some of the project teams about practical tips on how to increase continuity of care. We will finish the webinar by sharing how we plan to measure the impact of this work.
