Why it matters

GPs and general practice teams remain central to end of life care, providing essential support to patients with advanced and serious illnesses, and coordinating care across the wider health system. Yet we know that around one in four people each year still do not receive the end of life care and support they need. That gap reminds us why this work matters so deeply.

Thinking about taking part next year? Now’s the perfect time to begin.

  • Download the ‘How to Get Started’ guide - pick one priority area and build from there. Small, tailored changes make a big difference.
  • Explore the Daffodil Standards and tools – Practical guidance to help you plan, deliver and evaluate improvements.
  • Start a QI project – Choose a priority area for your practice and use our step by step approach.
  • Join training – Access webinars and resources tailored to GP teams.
  • Share your story – Case studies help spread what works and inspire others.
  • Nominate your team – Celebrate colleagues who go above and beyond.

2026 applications will open in late Spring.
