EOLC - What Matters Most: Important conversations for living and dying well

Having meaningful conversations around what matters to us is essential throughout life. Asking 'What Matters Most' with patients and families helps personal wishes be heard and understood. This is particularly important for those nearing the end of life and reframing Advance Care Planning conversations in this way can have a big impact on people's wellbeing as well as their care. ‘What Matters Most’ conversations support people to live a life that is meaningful and fulfilling for them. 'What Matters Most' conversations are often started between family and friends in social settings and when professionals become involved there's an opportunity for us to focus on life-affirming decision making - wider than medical treatment escalation. This conference is for you if you want to think about how you can do things differently and use these conversations in your practice.
