Lindsay Oliver (Year of Care Partnerships) will explain how the Year of Care approach to personalised care and support planning is being used to deliver proactive care​. Dr Ollie Hart Peak Health) will draw on his experience of training health coaching, care co-ordinators and link workers, to offer a perspective on how the personalised care team can support multi-mobility care.
