Cancer Care Review (CCR)

The Cancer Care Review (CCR) is carried out by the GP practice within six months of a diagnosis of cancer. Continued annually, it helps the person affected by cancer to understand what information and support is available to them, and enables supported self-management.

Holistic needs assessment and Care Planning

The Holistic Needs Assessment (HNA) is a simple questionnaire completed by the patient. Carrying out a HNA at key points of diagnosis and treatment ensures that people’s physical, practical and emotional concerns are identified and discussed.

A care and support plan can then be created in partnership with the patient. This may include lifestyle advice, information about self management, signposting to local support available or referral to specialist services.

Treatment Summary

The Treatment Summary was developed to improve communication between cancer services and primary care. Shared with the patient and their GP, the Treatment Summary provides a simple summary of a patient’s cancer diagnosis, treatment and ongoing care plan.

If a Treatment Summary has not been provided, consider requesting one to ensure a smooth transition for the patient from secondary to primary care.

Health and wellbeing events

Health and wellbeing events are education and information events for people living with cancer and their families. They should include information on: self management; healthy lifestyle; benefits and financial advice; possible consequences of treatment, and local services and support groups.

If you have a patient who you think would benefit from attending a health and wellbeing event please contact the relevant secondary care cancer care team or clinical nurse specialist.
