General Practitioners … are essential to the heart and soul of
medicine. It is increasingly realised that this development and
emancipation of general practice is not only a question of
professional pride and status but is an urgent economic need. Only
by developing a higher standard of general practice, and by making
full use of properly trained general practitioners
(with access to hospit
al and laboratory facilities), can the present overcrowding of
outpatient departments and excessive specialist consultations be
avoided."

Education Committee formed.
It contacted all the medical
schools in Britain asking whether there were any arrangements for
general practitioners to teach students general practice; how many
hours lectures on general practice took place; whether students
visited practitioners in their rooms and on rounds. The committee
published its findings in the BMJ 1953 [II: 36] Teaching of General
Practice by General Practitioners "We believe that the time has
come for medical schools and general practitioners to combine, in
.. mutual understanding, over the training of family doctors. Both
should recognise their responsibility for the quality of general
practice in this country."

Postgraduate
Education Committee formed.
This was to encourage
postgraduate instruction as a prerequisite for practice and
training a qualified doctor for a career in general practice;
continuing a practitioner's education throughout the working life,
encouraging specialist interests, providing a centre for
practitioners to meet and exchange ideas. It was hoped that the
college would be a clearing house for ideas, equipment and
information for general practitioners.

Working party on report
of Royal Commission on Medical Education [Todd
Commission]
Todd report recommended three years post registration "General
professional Training"; two further years "further
professional training"
"If there is to
be a programme of professional training for this branch of
medicine, extending over several years after primary qualification,
resources must be developed to meet new needs… the greatest need
will be for doctors who are capable of teaching, but willingness
and nascent ability are not sufficient. Unlike other branches of
medicine there is not an established tradition of teaching within
general practice itself…in the changing climate of practice the
single-handed doctor is becoming the exception rather than the
common-place. Group practice has obvious advantages for teaching
but it also poses problems; not only must individuals be selected
but also ‘teaching practices’… If growth [of the college] is to be
maintained and membership is to remain a desirable attainment for
doctors of the future, the College must be prepared and equipped to
examine much larger numbers than it has handled heretofore… this
will add new strains on its resources both in manpower and in
organisation."

"The College, it seems, should take a far more lively
interest in helping established general practitioners, especially
members, regularly to review their competence and thereby become
better doctors. More emphasis on clinical practice, continuing
education which is relevant to the everyday work of general
practice, and a commitment to the idea that the individual doctor
should be prepared to have his worked regularly assessed by his
peers, are the main inter-linked parts of this theme. Not
surprisingly therefore, there is a powerful ground swell of opinion
in favour of strengthening the faculties…" Ekke

They provide a link between individual health care and care for the

increasing role in addressing the health problems that we are

Adults [updated Jan 2006 by Professor Steve
Field, reviewed Jan 2007]
"It is one of the greatest
privileges of being a general practitioner to be able to practice
longitudinal care – caring for patients through a large part of
their lives and involving several generations of the same family
and their relatives. The general practitioner is put in the
position of trust and confidence in the care of the elderly and
dying relative. It is therefore important that the general
practitioner understands not only the clinical care of this group
of patients, but also how that care interacts with family, carers
and the multi-disciplinary team...."

RCGP statement on GP Training
We are very concerned that training budgets
are being cut – and the strong GP workforce of the future
jeopardised - in order to meet NHS deficits. We will be making
representations to Ministers about this. We understand that money
to make up the deficits must be found from somewhere but high
quality training for the GPs of the future must be safeguarded and
cannot be compromised, even in the short term.
The RCGP wants to see more invested in
training – and greater innovation in methods of training in primary
care. We urge Strategic Health Authorities to improve the
resourcing of training programmes for general practice. This is
essential to deliver the aims of the White Paper Our Health,
Our Care, Our Say and to improve patient care even
further.
RCGP Chairman Professor Mayur Lakhani said:
“We can understand the distress of junior
doctors who have had their GP training deferred. The RCGP is
determined to ensure that innovative specialist training for
general practice remains high on the agenda and will continue to
make representations to Government. We will also continue to
work with our colleagues on the GPC to ensure that GP workforce
issues are addressed by the NHS.”
