Application steps and checklist:

1. Download and complete an application form. Although you
will be able to complete the fields conveniently by typing in your
details, you will need to print and post your signed application
form to the membership services team.

2. Select a membership subscription
fee appropriate to your earnings and personal circumstances,
ensuring that you have checked the discounted rates that are on offer.
Remember if you are (re)joining during the membership
year, you will pay a pro-rata subscription fee and will be invitied
to renew on the following 1 April.

3. Select your preferred payment method. This can be a
cheque, Direct Debit, credit or debit card. Please note
payments by Direct Debit offer a further discount from the standard
rates.

|
4. Post your signed and completed application form along with
your payment (or payment forms) and a photocopy of your
JCPTGP/PMETB/GMC certificate to the membership services team for
processing.

Undertaking iMAP?
Please note that doctors undertaking iMAP will be awarded Associate membership
automatically upon submission of the application for iMAP and
eligibility for the programme has been confirmed. If you completed
iMAP prior to 1 May 2010 and would like to become a Member, please
complete and return the Member application form to the membership
services team.
Recently completed iMAP?
In order to upgrade to full Membership, please complete and
return a Direct Debit mandate form or a pro-rata subscription
fee payment to the membership support team, as per iMAP team's
instructions.
Medical students, Foundation programme doctors and GP
specialty trainees must register online through the New
Professionals portal.
