Forms

Doctors beginning training in August 2007 will not need to complete any forms as you can register and pay the initial subscription fee online via the College website.

Confirmation of your registration will be sent to you by email, containing a direct debit mandate for the annual subscriptions, which you will need to download, complete and return to the College by post (original signature required).
