The iMAP application form may be downloaded
using the link on this page.
Application for iMAP must be made via post.
When submitting your iMAP application form please ensure you
enclose all the requested documents and tick all relevant boxes on
the application form. Failure to do so will result in your
application being returned and will delay the process.
When submitting your application please ensure that you attach
sufficient postage for the size of envelope you are
sending. Failure to do so could result in delay or
non-delivery.
The Cardio-Pulmonary Resuscitation certificate
may be submitted at any time between application and initial
submission of the portfolio. This should be a full certificate (not
an update) dated within 12 months of application.
Candidates should declare any outstanding
complaints or clinical governance issues at the time of their
application and must submit the relevant details accordingly.
Candidates are advised to read carefully the
iMAP Candidate Regulations prior to applying to ensure they meet
the eligibility criteria.
