Using CARE measure
50 valid questionnaires should be
collected from consecutive patients attending routine
consultations. Reception staff should give the questionnaire to the
patient on arrival, and patients should complete it immediately
after seeing the doctor, and return it to reception. A sealed box
should be provided for this purpose.
The CARE measure
questionnaire and patient's cover letter are available to download
below:

questionnaire (Word, 39k)
Each doctor should then
personally or, if they are willing to give their username and
password, ask a member of staff to administer all 50 completed
questionnaires into the online CARE measure. The data will then
turn automatically into a report available to print or save.
Scoring
The scoring system for each item is:
'poor'=1,
'fair' = 2,
'good' = 3,
'very good' = 4, and
'excellent'= 5.
All ten items are then
added, giving a maximum possible score of 50, and a minimum of 10.
Up to two 'Not Applicable' responses or missing values are
allowable, and are replaced with the average score for the
remaining items. Questionnaires with more than two missing values
or 'Not Applicable' responses are removed from the analysis.
