Aim of the Award

This award recognises the outstanding work of GP Practice Teams in Scotland and the high standard of quality patient care they achieve.

How to Apply

Please provide a written description of an improvement you have implemented in clinical care that has been beneficial to your patients.

 

The improvement should involve input from all members of your practice team and should have taken place within the past 2 years. You should also include your list size and number of doctors employed and attached staff.

Assessment Criteria

In addition, please supply a statement of your Quality and Outcomes Framework points achieved in the last year. At least 585 points must be achieved in the clinical domain of the QOF for this period, ending 31st March 2009.

Judging Guidelines

The judging panel will consider the following questions during discussion:

  1. How many patients have been affected by the improvement?
  2. How has quality of life been affected as a result of the improvement?  For example, improvement in morbidity, loss of disability, improvement in mental well being.  
  3.  What is the likely impact on life expectancy?
  4. Is the improvement innovative?
  5. Have the team been involved in all stages of planning, implementing and evaluation?
  6. Have patients been consulted and involved?
  7. Can the improvement be replicated throughout Scotland?
  8. Is the presented material of a standard that can be published?
  9. Would a peer group think this improvement represents excellence in quality of care?
