The CARE Measure online tool is no
longer available for purchase.
The Patient Letter and Questionnaire are still
available to use and can be downloaded free of
charge from the "Using
CM" page.
Please note, however, that there will be
no report facility available and all questionnaires will need to be
collated manually.
